Hospital to medical home: Final report

This final report describes the Hospital to Medical Home project, designed to to maximize the health status and quality of life of children with life limiting and/or life threatening special health care needs by implementing a model for their safe transition from the acute care hospital setting to a qualified medical home. The report describes the purpose of the project and its relationship to Title V maternal and child health programs, the program's goals and objectives, methodology, evaluation, and outcomes. It also describes the products and publications produced during the project, the dissemination of the project results, and future plans for sustainability.
